Complete the patient information forms that were emailed to you. Have your medical insurance card with you to check in. Bring your glasses and current contact lens info along with an updated medication list. Upon arrival, call from the parking lot before coming in to let us know you are here and how we can help today.

Documents
Name of vision savings plan and medical health insurance plan, if you are planning to use a benefit during your visit. Full name, date of birth, and insurance subscriber's info, if different. If insurance subscriber is different than patient, provide info listed above.

During your exam you'll receive an interactive vision exam, comprehensive eye health assessment, discussion of visual concerns, and a prescription analysis.
